The Contract
"We" are MEDVISORY Ltd, a company registered in England and Wales under company number 06910164 and with our registered office at 4 Yorke Street, Hucknall, Nottingham, NG15 7BT.

"You" are the customer who has accessed our website and intends to place or has placed an order for a 15-minute telephone consultation, CDT blood test and phlebotomy (services) as a package or any combination of the above.
​
The purpose of the services is to give you a guide of your position as a High-Risk Offender (HRO) based on the information you have provided and considering DVLA standards for alcohol misuse.
​
This information along with your CDT blood test result will be used to provide you with a prediction of whether you are likely to pass (green), require more tests or lifestyle modification (amber) or fail (red) based on DVLA cutoffs for CDT testing and your drinking pattern.
​
We will provide an informed indication of the outcome with some degree of accuracy. We do not, however, provide any guarantees that you will pass the final DVLA HRO medical even if you may have passed our assessment.
You must be at least 18 years of age to have the tests.
​
During the order creation for the CDT blood test, you must provide the name, address and date of birth of the person having the test. These are the unique identifiers used when labeling the blood sample. A photograph identity will be undertaken to verify the correct person is undertaking the test.
​
As part of the service, you will be requested to complete a “Questionnaire” - a series of questions which allow an assessment to be made of your drinking habits and patterns.
​
You must complete truthfully all data fields of the questionnaire to allow the doctor to make an honest and transparent assessment to be later cross-referenced with your CDT test result.
If you fail to provide all the information in full in the questionnaire, the contracted consultation time may need an extension to obtain the missing data with additional charges incurred by you.

We will only send the test kits to UK addresses and process results for CDT blood samples taken and posted in UK.
​
If you choose to make your own phlebotomy arrangements, you must ensure that the person using the kit is a qualified phlebotomist. We accept no liability for the service you receive.
We will provide you with a pre-paid envelope with a Royal Mail tracking number. Please take note of this number. You are responsible for mailing the sample on the same day ideally in a Royal Mail Priority Post Box which is prioritized over the standard service. If this is not possible please keep your sample in a cool place overnight and post it the following day.

We aim to process the results and contact you by email, telephone or SMS within 96 hours of the laboratory receiving the sample. However, we are not responsible for any delays at the testing laboratory, postal delays or lost samples.
​
Based on the results of your test we may recommend a follow-up CDT blood test to monitor your results. We do not accept liability in the event you choose not to follow guidance.

Personal Information
We will not share the outcome of your assessment with us with the DVLA, your doctor or any third party.
​
We will not sell or pass on your personal information to third parties to enable them to contact you directly or market their products or services to you.
​
We may contact you after the formal DVLA HRO medical for the outcome as part of our quality control and audit.
​
Your information may also be used, on an anonymous basis, by us and third parties engaged by us for the purpose of scientific research or examining aggregate medical or clinical trial data.

Cancellation & Refund Policy
You have 21 days from the date of the online order (cancellation period) to cancel your test.
​
Cancellation must be done by email using the online contact form before midnight of the last day of the cancellation period.
We will consider exceptional circumstances outside the cancellation period at our discretion.
​
The following refunds apply within this cancellation period:

If your CDT test kit is yet to be dispatched - full refund.

Cancellation after your CDT test kit has been dispatched - full refund less £10 inc. VAT.

Cancellation after sample has been taken – no refund.

Failure to mail sample after sample has been taken – no refund.

Failure to attend pre-booked clinic and phlebotomy (blood sample) slot – no refund

Cancellation beyond 21-day cancellation period – no refund

Refunds will be paid using the original method of payment within 10 days. An alternative date with the same service may be offered.
​
Repeat Or Failed Sample Policy
In the event the sample has been damaged, is inadequate because of the phlebotomy service, we will offer a repeat sample at no cost to you. This will be communicated as soon as the laboratory informs us, and an alternative date offered. In the event of the sample being haemolysed in transit we will send a new blood kit to you free of charge. You will be responsible for any costs of phlebotomy if you choose to have the phlebotomy service done elsewhere. If your initial sample was obtained at our clinic network, the phlebotomy will be performed at no charge to you.

If the doctor, nurse or phlebotomist fails to obtain a sample you will be offered another test. If, however, they fail a second time the full cost of the test (£95) and phlebotomy (£30 inc. VAT) will be refunded in full.
Liability
Our liability to you is to the value of the service purchased.
​
In particular, we are not liable for any costs, loss, delay, inconvenience or damage you suffer as a result of the sample you send being lost, delayed or damaged in transit or via post from you to the laboratory; the laboratory being unable to test your sample for any reason; any injury, damage or loss you may suffer when opening (or handling or using the components contained in) a sample collection kit which you have been informed is to be used only by a qualified phlebotomist; any injury, damage or loss you may suffer as a result of your sample being collected by someone whom we have not referred you to or as a result of using equipment or sample collection components that we have not sent to you; test results not being made available to you within the turnaround time; your failure to act upon our advice if we recommend that you seek medical advice or attention having taken a test; your failure to send a sample to the laboratory for testing the same day.

Payment Methods
Pricing for all services is prominently displayed on the website. We accept payments via debit card, credit card and PayPal.
Time to time we may alter our pricing, but this will not affect you if you have already placed an order.
All payments must be cleared before any services are provided.

Consultations may be recorded for training and monitoring purposes.
